Information regarding Flu vaccination

Information regarding Influenza vaccination for newly inhabitants from foreign countries.
We’re opened for a person, above 7years old (including 6years old who have benn elementary school).

1 Non Reservation system
Please come to our clinic directly.
We recommend you to visit around after 4 o’clock till 5 in the afternoon.

2 Things to bring on the day and CHARGE
＊ID card (Driver’s License, My number card, Health insurance card)
　　　Patientｓ living in Kikuyo machi: available to use city subsidies when we check the address with one of ID cards above,which give you cost 1400yen for under 65 years old.

Patients living in Kumamoto city:bring ID card to register, Kumamoto city doesn’t support for flue vaccination charge. It charges 3,900yen for people younger than 65 years old.

＊Maternity handbook or Vaccination Record is needed, if the patient is under 18 years old.

(For gardians: Please ask town or city office if you haven’t had it yet.)

3 Quastionnare
We have prepared Flu questionnare in English and you will fill them in our clinic.

We’re happy to support non-Japanese speakers.
